Do you accept insurance?
PT Mindfully is an out-of-network provider with all insurance companies, however a superbill – or itemized receipt – can be provided upon request. Submit this to your insurance company for direct reimbursement of your out-of-network benefits. You may choose to pay with cash, check, or credit card on venmo or Paypal.
What do I need to bring?
Come to your appointment wearing clothes you can move in and allow access to the area of concern.
Bring any doctor referrals, imaging reports, or other pertinent documents to your first appointment.
Where are you located?
PT Mindfully services can be accessed remotely with Telehealth, or in-person visits can be arranged within the city of San Francisco and at Presidio Sport and Medicine. Additional fees may apply for in-person services outside of San Francisco. Please contact me for further information.
What is your cancellation policy?
We all get busy, sometimes even forgetful! However appointments missed also affect others who may need to be seen. Changes can be made to appointments prior to 24 hrs of the scheduled time, at no cost. Cancellations made without 24 hrs notice however are subject to 100% of the session fee. You may reschedule your visit due to illness, with no additional fee.
